Important Information For Future and Current Athletes

Thank you for your interest in being a part of the St. Francis athletic family! Below is some important information about sports participation and required paperwork to be a Shark! 

Concussion Testing
Any students interested in playing sports at St. Francis during the 2013-2014 school year must complete the ImPact Concussion Baseline test. This test will be offered through the IT Department at St. Francis and is available at the following times:

Monday, June 3rd between 2pm-4pm
Tuesday, June 4th between 2pm-4pm
Wednesday, June 5th between 2pm-4pm

These times are for walk-ins, and no appointment is necessary. No student-athlete will be able to participate in practices or games, including summer workouts and open gyms, unless they have taken the Baseline Test once in their high school career.

Student-athletes must bring a signed Concussion Monitoring Form with them before they are allowed to test. The form can be found here: Concussion Monitoring Form

If a student-athlete cannot make one of the walk-in times listed above, he/she must contact Head Football Coach, Adam Hazel for an individual appointment.

Required Paperwork for Student-Athletes
All student-athletes must also have on file a valid physical as well as the Athletic Participation Regulations Form. No student-athlete will be able to participate in practices or games, including summer workouts and open gyms, unless they have turned in both of these items to the Main Office.
